• Registration is done in person by appointment at the Chenango Valley Middle School/High School building.

• Appointments are available 7:30 a.m. to 3:30 p.m. Early or late times can be arranged if needed.

• Please bring a photo ID when you visit.

• Student's Original Birth Certificate  
• Two Proofs of Residency
• Guardian Photo Identification
• Immunization Records
• Custody Papers (if applicable)

• Lease (needs to be notarized)
• Recent Utility Bill (heat, electric, phone, cable, water, garbage)
• Mortgage Papers
• Valid Motor Vehicle Registration
• Valid Driver's License

• Absolutely! Please call 607-762-6821. With notice, we can also accommodate appointments outside of our regular hours, if it is necessary for you.

• During your appointment, our staff will review your documents, answer questions, and help you complete any remaining forms.
   

• If your family has moved, please let us know your new address.
• You do not need to fill out a new registration form. Please complete the Change of Address Form instead.
• You’ll need to show two proofs of your new address. You can use the same types of documents listed above.

    • Bringing them to the security office at CV Middle/High School for copies to be made and sent to Registration.
